1. Open an Internet browser and enter the URL: http://adulted.otan.us/tp/assessment/ or click here
2. Enter the password: smace
3. Type in your names correctly and choose your position. Click Save.
4. Follow the instructions and answer all questions.
5. Log out after you have completed the survey.
